Coordination and Networking for Healthcare (in the Border Region)

CONNECT is part of the larger project “Intersectoral Care of Vulnerable Groups (INGVER), funded by the Federal Ministry of Education and Research (BMBF). INGVER is dedicated to improving the healthcare of three vulnerable groups (high risk infants, cancer patients, and geriatric patients) across health service sectors and care settings with a personalized and interdisciplinary approach.

The goal of CONNECT is to examine specific healthcare challenges and needs within these groups in the northern German-Dutch border region. Rural border regions, such as the Ems Dollart Region, are characterized by a unique set of challenges when it comes to healthcare, including inadequate service infrastructure and limited or lacking access to service options across the border.

CONNECT will address these issues by mapping out existing service and support structures, identifying stakeholder perspectives on care pathways and options, and assessing the intersectoral linkages in health data flow between care domains and settings to eventually enhance the delivery and quality of healthcare in the region.
