Cross-sectoral care for vulnerable groups – INGVER
The ‘Intersectoral Care for Vulnerable Groups – INGVER’ project at University Medicine Oldenburg (UMO) is dedicated to improving care for health-vulnerable groups through a cross-sectoral, personalised and interdisciplinary approach. These groups – including those at risk from birth, cancer patients and older people – have specific, sometimes complex medical and psychosocial needs that require intensive and personalised care. All three vulnerable groups share several aspects relevant to their care, such as chronic and psychiatric conditions, comorbidities, resilience and coping with critical life stages, as well as the importance of family and long-term care.
The main objective of the project is to develop innovative diagnostic, treatment and care concepts based on personalised medicine. Particular emphasis is placed on cross-sectoral networking, i.e. collaboration between inpatient and outpatient facilities and home care services.
A key element of the project is the development of a diagnostics and data integration centre that continuously collects and integrates data. The aim is to incorporate cross-sectoral information into personalised treatment plans to ensure seamless and interdisciplinary care. In addition, the border region between Germany and the Netherlands is being utilised to compare the strengths of both healthcare systems and to derive cross-border care models from this comparison.
