    @article{Diekmann.2022, abstract = {BACKGROUND When older adults fall below the thresholds of functional geriatric assessment (FGA), they may already be at risk of mobility impairment. A reduction in (jumping) power could be an indication of functional decline, one of the main risk factors for falls. OBJECTIVE This paper explores whether six-month delta (∆) values of muscle power can predict 24-month follow-up FGA in older adults. METHODS This observational study of independent, healthy, high-performing community-dwelling adults aged 70 + years involved FGA (mobility, balance, and endurance tests) at baseline (t0), after 6~months (t1), and after 24~months (t2); maximum jumping power (max JP) was determined at t0 and t1. A predictive linear model was developed in which the percentage change of \textgreek{D}max JP0,1 was transferred to all FGA (t0) values. The results were compared with measured FGA values at t2 via sensitivity and specificity in terms of the clinically meaningful change (CMC) or the minimal detectable change (MDC). RESULTS In 176 individuals (60{\%} female, mean age 75.3~years) the mean percentage (SD) between predicted and measured FGA ranged between 0.4 (51.3) and 18.11 (51.9). Sensitivity to identify the CMC or MDC of predicted FGA tests at t2 ranged between 17.6{\%} (Timed up and go) and 75.0{\%} (5-times-chair-rise) in a test-to-test comparison and increased to 97.6{\%} considering clinically conspicuousness on global FGA. CONCLUSION The potential of jumping power to predict single tests of FGA was low regarding sensitivity and specificity of CMC (or MDC). 6~months \textgreek{D}max JP seem to be suitable for predicting physical function, if the measured and predicted tests were not compared at the test level, but globally, in the target group in the long term.},

    @article{Elfert.2022, abstract = {Due to the physical, psychological, or socioeconomic changes that accompany aging, many people will be affected by geriatric frailty syndrome, which can lead to multimorbidity and premature death. Nutrition counseling is often used to prevent and intervene in frailty syndrome, especially in geriatric rehabilitation. To this end, the consumption behavior of geriatric patients is recorded using paper-based, as well as retrospective memory logs in face-to-face interviews between patients and nutritionists. To simplify this procedure, a digital nutrition diary was developed that is specially adapted to the needs of geriatric patients ({\&}gt;=70 years), enabling them to record their consumption behavior themselves. In an initial study (Study 1), conducted in a geriatric rehabilitation division with twelve subjects (ten male, two female, mean age 79.2 $\pm$5.9 years), feedback about the usability of the digital nutrition diary, and how to improve it, was surveyed. In addition, the usability of an activity tracker and a body composition scale was surveyed to determine whether geriatric patients are generally able to use these devices. In a second study (Study 2), also conducted in the geriatric rehabilitation division, this time with sixteen subjects (ten male, six female, mean age 79.3 $\pm$3.9 years), the usability of the digital nutrition diary was surveyed again to evaluate its modifications based on the feedback from Study 1. In Study 1, the usability rating of the system (0-100) was 82.5 for the activity tracker, 29.71 for the body composition scale, and 51.66 initially for the digital nutrition diary, which increased to 76.41 in Study 2.},

    @article{Fudickar.2022, abstract = {Comprehensive measurements are needed in older populations to detect physical changes, initiate prompt interventions, and prevent functional decline. While established instruments such as the Timed Up and Go (TUG) and 5 Times Chair Rise Test (5CRT) require trained clinicians to assess corresponding functional parameters, the unsupervised screening system (USS), developed in a two-stage participatory design process, has since been introduced to community-dwelling older adults. In a previous article, we investigated the USS's measurement of the TUG and 5CRT in comparison to conventional stop-watch methods and found a high sensitivity with significant correlations and coefficients ranging from 0.73 to 0.89. This article reports insights into the design process and evaluates the usability of the USS interface. Our analysis showed high acceptance with qualitative and quantitative methods. From participant discussions, suggestions for improvement and functions for further development could be derived and discussed. The evaluated prototype offers a high potential for early detection of functional limitations in elderly people and should be tested with other target groups in other locations.},

    @article{Brinkmann.2022, abstract = {Manual patient handling is one of the most significant challenges leading to musculoskeletal burden among healthcare workers. Traditional working techniques could be enhanced by innovations that can be individually adapted to the physical capacity of nurses. We evaluated the use of a robotic system providing physical relief by collaboratively assisting nurses in manual patient handling tasks. By quantifying kinetic and muscle activity data, it was possible to distinguish two kinds of movement patterns. Highly asymmetric postures and movements corresponded to distinct extremes in lower limb and spine muscle activity data. The use of collaborative robotics significantly reduced maximum force exertion in the caregiving process by up to 51{\%}. Lateral flexion and torsion of the trunk were reduced by up to 54{\%} and 87{\%},
      respectively, leading to a significant reduction in mean spine muscle activity of up to 55{\%}. These findings indicate the feasibility of collaborative robot-assisted patient handling and emphasize the need for future individual intervention programs to prevent physical burden in care.},

    @article{Happe.2022, abstract = {BACKGROUND Maintaining nutrition and exercise strategies after rehabilitation can be difficult for older people with malnutrition or limited mobility. A technical assistance system such as an e-coach could help to positively influence changes in dietary and exercise behavior and contribute to a sustainable improvement in one's nutrition and mobility status. Most apps do not provide a combination of nutrition and exercise content. In most cases, these apps were evaluated with healthy individuals aged {\textless}70 years, making transferability to vulnerable patients, with functional limitations and an assumed lower affinity for technology, in geriatric rehabilitation unlikely. OBJECTIVE This study aims to identify the potential for optimization and enhance usability through iterative test phases to develop a nutrition and mobility e-coach suitable for older adults ($\geq$65 years) based on individual health behavior change stages in a rehabilitation setting. METHODS Iterative testing was performed with patients aged $\geq$65 years in a rehabilitation center. During testing, participants used an e-coach prototype with educational elements and active input options on nutrition and mobility as a 1-time application test. The participants performed navigation and comprehension tasks and subsequently provided feedback on the design aspects. Hints were provided by the study team when required, documented, and used for improvements. After testing, the participants were asked to rate the usability of the prototype using the System Usability Scale (SUS). RESULTS In all, 3 iterative test phases (T1-T3) were conducted with 49 participants (24/49, 49{\%} female; mean 77.8, SD 6.2 years). Improvements were made after each test phase, such as adding explanatory notes on overview screens or using consistent chart types. The use of the user-centered design in this specific target group facilitated an increase in the average SUS score from 69.3 (SD 16.3; median 65) at T1 to 78.1 (SD 11.8; median 82.5) at T3. Fewer hints were required for navigation tasks (T1: 14.1{\%}; T2: 26.5{\%}; T3: 17.2{\%}) than for comprehension questions (T1: 30.5{\%}; T2: 21.6{\%}; T3: 20{\%}). However, the proportion of unsolved tasks, calculated across all participants in all tasks, was higher for navigation tasks (T1: 0{\%},
      T2: 15.2{\%},
      T3: 4.3{\%}) than for comprehension tasks (T1: 1.9{\%},
      T2: 0{\%},
      T3: 2.5{\%}). CONCLUSIONS The extensive addition of explanatory sentences and terms, instead of shorter keywords, to make it easier for users to navigate and comprehend the content was a major adjustment. Thus, good usability (SUS: 80th-84th percentile) was achieved using iterative optimizations within the user-centered design. Long-term usability and any possible effects on nutritional and physical activity behavior need to be evaluated in an additional study in which patients should be able to use the e-coach with increasing independence, thereby helping them to gain access to content that could support their long-term behavior change.},

    @article{Friedrich.2021, abstract = {Malnutrition is a well-known risk factor for deteriorated physical function, disability and loss in independence in older adults. An unintended loss in body weight of more than 5{\%} in 3 months is one indicator for malnutrition. In this study we examined the relationship between meal preparation time, hand grip strength, and body weight in order to map impending nutritional problems using ambient sensors. Data were collected in the domestic environments of 20 (pre-) frail older adults aged 85.75 y (Standard Deviation: 5.19 y) over 10-months of observation. Collecting included physical function and nutritional status of the participants and meal preparation time by a combination of motion and power sensor events. Analysis was done by rank correlation of hand grip strength, body weight, and meal preparation time. Ten participants aged 85.1 years (Standard Deviation: 4.6 y) were included. The results show a significant correlation ($\geq$0.99) of the meal preparation time with the hand grip strength. This result validated the meal preparation time as a suitable measure for analysing the correlation between meal preparation time and body weight, and a significant correlation ($\geq$0.99) found. Hence the meal preparation time could be used as an indicator for malnutrition. However, causalities have to be conducted by further clinical studies.},

In this prospective cohort study, patients admitted into geriatric rehabilitation were consecutively included between September 2015 and September 2016, aged $\geq$70 years. Individual activities of daily living were documented by the Barthel index (BI, 0-100 points). Obesity was assessed by the measurement of body mass index (BMI, kg/m$^2$), waist circumference (WC, cm) and percentage of body fat mass ({\%}FM) based on triceps' skinfold thickness at admission (t1), discharge (t2) and six months after discharge (t3). A total of 122 patients were included in the analysis. Prevalence of obesity according to BMI, WC and {\%}FM was 33.6{\%},
      83.6{\%} and 71.3{\%} respectively. Patients with a high WC and patients with a high BMI had lower BI values at t1, t2, t3 and the improvement in BI (t1-t2, t2-t3) was lower than in those with low WC and low BMI, but without statistical significance. In multiple regression analysis, BMI, WC and {\%}FM were not associated with BI at t3 and improvement of BI (t2-t3). Obesity was highly prevalent in geriatric rehabilitation patients, but it was not associated with BI during the 6-month follow-up.},

    @article{Diekmann.2021, abstract = {BACKGROUND It is important to identify the relevant parameters of physical performance to prevent early functional decline and to prolong independent living. The aim of this study is to describe the development of physical performance in a healthy community-dwelling older cohort aged 70+ years using comprehensive assessment over two years and to subsequently identify the most relevant predictive tests for physical decline to minimize assessment. METHODS Physical performance was measured by comprehensive geriatric assessment. Predictors for the individual decline of physical performance by Principal Component and k-means Cluster Analysis were developed, and sensitivity and specificity determined accordingly. RESULTS 251 subjects ({\O} 75.4~years) participated in the study. Handgrip strength was low in 21.1{\%}. The follow-up results of tests were divergent. Handgrip strength [-~16.95 (SD 11.55)] and the stair climb power test (power) [-~9.15 (SD 16.84)] yielded the highest percentage changes. Four most relevant tests (handgrip strength, stair climb power time, timed up {\&} go and 4-m gait speed) were identified. A predictor based on baseline data was determined (sensitivity 82{\%},
      specificity 96{\%}) to identify subjects characterized by a high degree of physical decline within two years. DISCUSSION Although the cohort of older adults is heterogeneous, most of the individuals in the study exhibited high levels of physical performance; only a few subjects suffered a relevant decline within the 2-year follow-up. Four most relevant tests were identified to predict relevant decline of physical function. CONCLUSION In spite of ceiling effects of the geriatric assessment in high-performers, we assume that it is possible to predict an individual's risk of physical decline within 2~years with four tests of a comprehensive geriatric assessment.},

    @article{Happe.2021, abstract = {BACKGROUND AND OBJECTIVE Restrictions in public life during the COVID-19 pandemic mainly addressed the safety of high-risk older multimorbid patients to protect them from infection. Nevertheless, detrimental aspects of the lockdown for older people are a~reduction in physical activity and limited access to physiotherapy, which are likely to have a~negative impact on the health status. This study examined the feasibility of video-based physiotherapy (VT) and the subjective rating of VT by patients and therapists. METHODS From April to June 2020, 4 facilities with 9 patients (6~women, 64-82~years old) participated in the study and were provided with technical equipment. Semistructured telephone interviews were conducted in eight patients to assess physical activity, functional ability and participation before and during the restrictions. Both patients and therapists were asked about their subjective experiences with VT. RESULTS A total of 92~VT sessions took place. Performance of the exercises and comprehension of instructions were rated as good to very good. The level of acceptance was high. Of the patients four described a~perceived reduction in their physical activities due to the public restrictions; however, in the questionnaires no substantial differences in participation and physical activity were found. DISCUSSION The use of VT is feasible in older patients. Only minor technical support is needed. In pandemic situations or in other contexts VT appears to be a~promising supplement or alternative to normal physiotherapy. Further studies are needed to identify suitable patient groups, to prove efficacy and to develop further content-related aspects of VT. ZUSAMMENFASSUNG HINTERGRUND UND ZIEL: Einschr{\"a}nkungen des {\"o}ffentlichen Lebens durch die COVID-19-Pandemie dienen insbesondere dazu, Risikogruppen vor einer Ansteckung zu sch{\"u}tzen. Darunter fallen auch {\"a}ltere, multimorbide Patienten, f{\"u}r die k{\"o}rperliche Inaktivit{\"a}t und Auslassen von Ma{\ss}nahmen wie Physiotherapie jedoch negative Folgen haben k{\"o}nnen. Die vorliegende Studie untersucht die Machbarkeit und die subjektive Bewertung videobasierter Physiotherapie (VT). METHODEN Von April bis Juni 2020 nahmen 4~Einrichtungen mit 9~Patienten (6~Frauen, 64 bis 82~Jahre) an der Studie teil, die mit Tablets ausgestattet wurden. Durch semistrukturierte Telefoninterviews wurden k{\"o}rperliche Aktivit{\"a}t, funktionelle Kompetenz und Partizipation vor und w{\"a}hrend den Einschr{\"a}nkungen bei 8 Patienten erfasst. Patienten und Therapeuten wurden zu ihren subjektiven Erfahrungen mit der VT befragt. ERGEBNISSE Es fanden insgesamt 92~VT-Einheiten statt. Die Umsetzung der {\"U}bungen wurde als gut bis sehr gut bewertet. Insgesamt zeigte sich eine hohe Akzeptanz der VT. Vier von 8 Patienten beschrieben eine subjektive Reduzierung ihrer k{\"o}rperlichen Aktivit{\"a}ten aufgrund der Einschr{\"a}nkungen. Diese Ver{\"a}nderungen wurden {\"u}ber die verwendeten Fragebogen zur Partizipation und zur k{\"o}rperlichen Aktivit{\"a}t nicht abgebildet. DISKUSSION Bei {\"a}lteren Patienten ist VT mit geringer technischer Unterst{\"u}tzung machbar. Sowohl in Pandemiesituationen als auch in anderen Kontexten stellt sich VT als eine realisierbare Erg{\"a}nzung oder Alternative zur normalen Physiotherapie dar. Weitere Studien zur Identifikation von geeigneten Patientengruppen, Effektivit{\"a}t der VT und Weiterentwicklung inhaltlicher Aspekte sind dringend notwendig.},

In order to maintain these improvements in the medium- and long-term, behavioural changes regarding mobility and nutrition are also targeted, but these are often not sustainable. International studies show positive effects on the sustainability of the improvement of physical activity behaviour in healthy seniors through the use of electronic devices and software applications. Comparable approaches that include nutrition topics or combine them and were additionally developed for geriatric rehabilitation patients ($\geq$70 years) to date are not known. The aim of this study was to identify what geriatric rehabilitation patients require from an electronic coaching system (e-coach) to support them in improving their nutritional and physical activity behaviour, and what content and features physiotherapists and dieticians consider relevant. METHOD Focus group interviews (09-11/2019) were conducted in a geriatric rehabilitation centre in Germany with patients aged 70 years and older, relatives and experts (physiotherapists and nutritionists). The focus groups were recorded, transcribed verbatim and analysed using content analysis. RESULTS Three focus groups with patients and relatives (n = 17, 65{\%} female, 16 (94{\%}) in age category 70-99 years) and one focus group with experts (2 dieticians and 1 physiotherapist) were conducted. Relevant contents and feedback elements for nutrition and physical activity in old age were identified. The patients' comments show that an e-coach must offer obvious benefits for the older persons and promote motivation in order to be used. The willingness to change nutrition and physical activity behaviour and the previous experiences in these areas are very heterogeneous, therefore content should be adaptable to different requirements. CONCLUSION Experts and patients identified quite similar contents, barriers and facilitators for a nutrition and physical activity e-coach. The e-coach needs to be able to address different points of behaviour change, enable adaptations to the individual patient and convince the older person that using it will help them to improve their nutrition and physical activity. It is also important that the e-coach is easy to use and can be easily integrated into the patient's everyday life after rehabilitation.},

Established instruments like the Timed {\textquotedbl}Up {\&} Go{\textquotedbl} (TUG) Test and the Sit-to-Stand Test (SST) require a trained person (e.g., physiotherapist) to assess physical performance. More often, these tests are only applied to a selected group of persons already functionally impaired and not to those who are at potential risk of functional decline. The article introduces the Unsupervised Screening System (USS) for unsupervised self-assessments by older adults and evaluates its validity for the TUG and SST. The USS included ambient and wearable movement sensors to measure the user's test performance. Sensor datasets of the USS's light barriers and Inertial Measurement Units (IMU) were analyzed for 91 users aged 73 to 89 years compared to conventional stopwatch measurement. A significant correlation coefficient of 0.89 for the TUG test and of 0.73 for the SST were confirmed among USS's light barriers. Correspondingly, for the inertial data-based measures, a high and significant correlation of 0.78 for the TUG test and of 0.87 for SST were also found. The USS was a validated and reliable tool to assess TUG and SST.},

The present study aimed to identify setting-specific risk profiles of malnutrition in older adults and to investigate the association of the number of individual risk factors with malnutrition. DESIGN Data of four cross-sectional studies were harmonized and uniformly analysed. Malnutrition was defined as BMI {\textless} 20 kg/m2 and/or weight loss of {\textgreater}3 kg in the previous 3-6 months. Associations between factors of six domains (demographics, health, mental function, physical function, dietary intake-related problems, dietary behaviour), the number of individual risk factors and malnutrition were analysed using logistic regression. SETTING Community (CD), geriatric day hospital (GDH), home care (HC), nursing home (NH). PARTICIPANTS CD older adults (n 1073), GDH patients (n 180), HC receivers (n 335) and NH residents (n 197), all $\geq$65 years. RESULTS Malnutrition prevalence was lower in CD (11 {\%}) than in the other settings (16-19 {\%}). In the CD sample, poor appetite, difficulties with eating, respiratory and gastrointestinal diseases were associated with malnutrition; in GDH patients, poor appetite and respiratory diseases; in HC receivers, younger age, poor appetite and nausea; and in NH residents, older age and mobility limitations. In all settings the likelihood of malnutrition increased with the number of potential individual risk factors. CONCLUSIONS The study indicates a varying relevance of certain risk factors of malnutrition in different settings. However, the relationship of the number of individual risk factors with malnutrition in all settings implies comprehensive approaches to identify persons at risk of malnutrition early.},

This systematic review summarizes the evidence on nutritional and functional outcomes of nutritional interventions alone or in combination with physical exercise in geriatric rehabilitation patients. DESIGN Eight electronic databases were searched until July 1, 2019 to identify nutritional intervention studies in patients aged $\geq$60~years who were admitted to geriatric rehabilitation. A meta-analysis was performed to quantify intervention effects on serum albumin, muscle mass, and hand grip strength (HGS). RESULTS A total of 1962 studies were screened and 13 included in the systematic review. Studies were heterogeneous in interventions (4 nutritional interventions, 6 physical exercise~+~nutritional intervention, 1 timing of protein provision, 1 exercise~+~dietary advice, 1 nutrition-related nursing care) and outcomes. Among the 9 interventions that tested oral nutritional supplements (ONS) with protein, with or without exercise, 7 studies reported protein intake and 6 showed increased protein intakes, 2 of 5 studies showed increased albumin levels, and 5 of 9 reported an improvement in functional outcomes (BI, Functional Independence Measure, mobility). Meta-analyses showed no significant intervention effects on albumin [standardized mean difference (SMD) 0.45, 95{\%} confidence interval (CI)~-0.14, 1.04 (4 studies)], muscle mass [mean difference (MD) 2.14~kg, 95{\%} CI -2.17, 6.45 (3 studies)], and HGS [SMD -0.04, 95{\%} CI -0.55, 0.63 (3 studies)], but was based on a very limited number of studies. CONCLUSIONS AND IMPLICATIONS Only a limited number of studies with heterogeneous nutritional interventions and outcomes were available in the geriatric rehabilitation population. Studies that included ONS improved nutritional outcomes, especially protein intake and albumin levels. Functional outcomes improved in the majority of reporting studies. This indicates benefits of protein supplementation, with or without exercise, in this population. Future well-designed and well-powered clinical trials are needed to clarify existing controversial aspects.},

METHODS Eight databases were searched for full-text articles reporting baseline nutritional intake and status of adults $\geq$ 60~years in rehabilitation settings. Pooled estimates were calculated for prevalence of malnutrition and risk of malnutrition based on the Mini~Nutritional Assessment (MNA) and for mean body mass index (BMI). Associations between nutritional status (MNA, MNA short form and BMI) and functional status (Barthel Index and Functional Independence Measure) and prevalence of sarcopenia were reviewed. RESULTS 62 out of 1717 references were eligible for inclusion. Pooled prevalence [95{\%} confidence interval (CI)] of malnutrition and risk of malnutrition were 13 (5-20) {\%} and 47 (40-54) {\%}. Pooled estimate (95{\%} CI) for BMI was 23.8 (23.2-24.5) kg/m2. Existing data suggest a risk for low protein and energy intake and vitamin D deficiency. Functional status differed widely. Seven out of ten studies reported significant associations between reduced nutritional status and reduced functionality, whilst two out of seven studies reported significant associations between higher BMI and functionality. Prevalence of sarcopenia was high with 40-76{\%} in this population. CONCLUSIONS Although geriatric rehabilitation populations and settings were heterogeneous, a relevant percentage of geriatric rehabilitation patients were affected by a reduced nutritional status. Nutritional status was associated with decreased functionality. This emphasizes the need for screening for malnutrition and targeted nutritional intervention.},

In order to assure such an early detection, functional assessments must be conducted on a frequent and regular basis. Since the five time chair rise test (5CRT) is a well-established test in the geriatric field, this test should be supported by technology. We introduce an approach that automatically detects the execution of the chair rise test via an inertial sensor integrated into a belt. The system's suitability was evaluated via 20 subjects aged 72$^-$89 years (78.2 $\pm$ 4.6 years) and was measured by a stopwatch, the inertial measurement unit (IMU), a Kinect{\circledR} camera and a force plate. A Multilayer Perceptrons-based classifier detects transitions in the IMU data with an F1-Score of around 94.8{\%}. Valid executions of the 5CRT are detected based on the correct occurrence of sequential movements via a rule-based model. The results of the automatically calculated test durations are in good agreement with the stopwatch measurements (correlation coefficient r = 0.93 (p {\textless} 0.001)). The analysis of the duration of single test cycles indicates a beginning fatigue at the end of the test. The comparison of the movement pattern within one person shows similar movement patterns, which differ only slightly in form and duration, whereby different subjects indicate variations regarding their performance strategies.},

Due to its sensitivity regarding the indication of Parkinson's disease (PD) or increased fall risk in elderly people, this assessment test becomes increasingly relevant, should be automated and should become applicable for unsupervised self-assessments to enable regular examinations of the functional status. With Inertial Measurement Units (IMU) being well suited for automated analyses, we evaluate an IMU-based analysis-system, which automatically detects the TUG execution via machine learning and calculates the test duration. as well as the duration of its single components. The complete TUG was classified with an accuracy of 96{\%} via a rule-based model in a study with 157 participants aged over 70 years. A comparison between the TUG durations determined by IMU and criterion standard measurements (stopwatch and automated/ambient TUG (aTUG) system) showed significant correlations of 0.97 and 0.99, respectively. The classification of the instrumented TUG (iTUG)-components achieved accuracies over 96{\%},
      as well. Additionally, the system's suitability for self-assessments was investigated within a semi-unsupervised situation where a similar movement sequence to the TUG was executed. This preliminary analysis confirmed that the self-selected speed correlates moderately with the speed in the test situation, but differed significantly from each other.},

The term 'geriatric rehabilitation' is wide and includes inpatient and ambulatory care as well as rehabilitation programs in hospitals, also outside geriatric wards. The role of nutrition is therefore not clearly defined, but an association between declined functional status and low nutritional status is evident. RECENT FINDINGS An association has been identified between malnutrition and low physical function or lower rehabilitation effect, respectively. In intervention trials with nutritional care (additional energy or protein), a number of benefits for muscle mass, muscle strength, physical function, or the outcomes quality of life and rehospitalization were identified. In this context, the combination of exercise and adequate nutrition seems to be beneficial. SUMMARY Geriatric patients undergoing a rehabilitation program outside inpatient, ambulatory, or hospitalization care need an adequate supply of energy and nutrients. Malnutrition must be avoided, ideally before starting rehabilitation. Nutritional interventions are most beneficial in combination with exercise training.},

Recently, significantly reduced anti-influenza antibody titers and increased rates of influenza infection after vaccination were reported in community-dwelling American frail older adults. The aim of our study was to further assess the relative impact of frailty and of each individual Fried frailty criterion on influenza vaccine response. Prefrail and frail community-dwelling German persons aged $\geq$70~years were recruited for a nutritional randomized double-blind placebo-controlled clinical trial conducted during the 2014-2015 influenza season. Herein, we present a sub-analysis study of the placebo group to compare 76 prefrail and frail participants. RESULTS Previous seasonal influenza vaccination rate was relatively high (77.6{\%}) in the 76 volunteers aged from 70 to 93~years. Of these participants, 65.8{\%} were diagnosed as prefrail and 34.2{\%} as frail according to the Fried frailty criteria. In both prefrail and frail groups, elevated levels of pre-vaccination seroprotection were observed to all vaccine strains (H1N1: 54{\%} and 32{\%},
      H3N2: 60{\%} and 72{\%},
      B: 10{\%} and 16{\%}). Post-vaccination, similar increases in haemagglutination-inhibiting antibody titers were observed for the three vaccine strains in both prefrail and frail groups. No significant difference in geometric mean titer (GMT) ratios and in rates of seroconversion or seroprotection were observed between prefrail and frail groups. Regarding the five Fried frailty criteria, only participants with low physical activity had significantly lower GMT to the strains H3N2 (55.4 vs 103.7, p~=~0.001) and B (13.9 vs 20.0, p~=~0.06), as compared to those having normal physical activity. CONCLUSIONS Influenza vaccine response was not significantly affected by the frail phenotype, as defined by Fried frailty criteria, in community-dwelling German individuals. However, there has been some uncertainty whether these instruments can describe the entire spectrum of mobility with adequate precision and whether they can be successfully applied for repeated assessments during rehabilitation. Against this background the De Morton Mobility Index (DEMMI) has been developed and validated. OBJECTIVE The aim of this study was to evaluate the suitability of the DEMMI compared to the TUG and Tinetti tests in early geriatric rehabilitation. MATERIAL AND METHODS Ceiling and floor effects of the DEMMI, the TUG test, the Tinetti test and the Barthel Index (BI) were determined. Correlations were calculated using the Spearman correlation coefficient. Effects on the DEMMI were estimated through multiple linear regression. RESULTS A total of 144 patients with an average age of 84.91 years (SD $\pm$5.77) and 65.97 {\%} females participated in this study. The mean value for the DEMMI was 33.45 (SD $\pm$15.37) points at admission and 43.90 (SD $\pm$15.55) at discharge (n~= 144). Floor effects were observed in the TUG test in 84.72 {\%} (n~= 122) of patients at admission and in 60.42 {\%} (n~= 87) at discharge. The DEMMI, Tinetti test and BI showed no floor or ceiling effects. The correlation between the DEMMI and the TUG test was -0.48 at admission and -0.49 at discharge, between the DEMMI and the Tinetti test 0.75 and 0.82, respectively and between the DEMMI and the BI 0.54 and 0.66, respectively (p~{\textless}~0,01). CONCLUSION Significant correlations with the TUG test, the Tinetti test and the BI, together with the lack of floor and ceiling effects provide evidence that the DEMMI is an appropriate tool to assess mobility in early geriatric rehabilitation. The lack of floor and ceiling effects appears to make the DEMMI a superior tool compared to the TUG test.},

Aim of this cross-sectional study was, first, to assess disease burden and nutritional status, denoted in anthropometrics, and, second, to investigate associations between anthropometrics and disease burden. DESIGN Cross-sectional multi-centre study. SETTING Home-care receivers living in three urban areas of Germany in 2010. PARTICIPANTS 353 elderly ({\textgreater}64 years) in home care (128 males aged 79.1 $\pm$7.8 years, 225 females aged 82.0 $\pm$7.5 years). MEASUREMENTS Nutritional status was assessed by body mass index (BMI), mid upper arm circumference (MUAC) and calf circumference (CC). Medical conditions were assessed in personal interviews. A 3-day prospective nutrition diary was kept. Metric data are reported as mean$\pm$SD or median (interquartile range), p{\textless}0.05 was considered significant. RESULTS Most participants were substantially (59{\%}), and 11{\%} severest in need of care. The seniors suffered from 5 (4-7) chronic diseases; dementia, depression, stroke, and respiratory illness were most prevalent (each 20-40{\%}). More than one-third of participants had only moderate or poor appetite, nearly half were unable to eat independently. Chewing problems were reported for 52{\%} of study participants, and more than one quarter of elderly had swallowing problems. Daily mean energy intake was 2017$\pm$528 kcal in men (n=123) and 1731$\pm$451 kcal in women (n=216; p{\textless}0.001). Mean protein intake amounted to 1.0 g/kg body weight. Mean BMI was 28.2$\pm$6.2 kg/m$^2$ (n=341), 14{\%} of seniors had a BMI {\textless}22 kg/m$^2$ (including 4{\%} with BMI {\textless}20 kg/m$^2$). Critical MUAC ({\textless}22 cm) was indicated in 6{\%} of subjects; and CC {\textless}31 cm in 11{\%} of men, 21{\%} of women (p{\textless}0.05). After adjusting for sex and age, BMI, MUAC and CC were negatively associated with high care level, hospitalization in the previous year, nausea/vomiting, prevalence of dementia, poor appetite, and eating difficulties like dependency, chewing and swallowing problems. CONCLUSION We recommend to pay special attention to the nutritional status of elderly persons in home-care exhibiting named disease burden.},

The BMI range used by the World Health Organization (WHO) to classify overweight, a~BMI of 25-30~kg/m2, can be regarded as normal weight in old age; nevertheless, obesity is associated with an increased risk of disability and of a~deterioration in physical functionality, particularly among older age groups. This relationship to obesity has also been established for frailty. For this reason, a~reduction in weight may be appropriate under functional aspects if BMI values exceed 30~kg/m2; however, such a~decision cannot be made on the basis of an individual BMI alone. The functional status, body composition, comorbidities and, in particular the life perspectives of the patient should also be taken into consideration. If weight loss is intended, it must always be performed under strict medical supervision involving optimized protein intake, a~carefully calculated moderate reduction in calories and adequate physical training. In the case of chronically ill elderly patients, weight reduction is not usually appropriate. Restrictive diets of any kind should principally be critically viewed in old age because even temporary inadequate energy intake may lead to accelerated deterioration of muscle and bone structure.},

A special focus has been placed on the effects on muscle protein synthesis, strength and overall performance in this population. RECENT FINDINGS Although for older adults, some additional evidence on the benefits of a higher protein intake than 0.8 g/kg body weight per day has been provided, the results of studies focusing on the timing of protein intake over the day have been contradictory. Supplementation with so-called 'fast' proteins, which are also rich in leucine, for example whey protein, proved superior with regard to muscle protein synthesis. First studies in frail older persons showed increased strength after supplementation with milk protein, whereas the combination with physical exercise increased muscle mass without additional benefit for strength or functionality. SUMMARY Recent evidence suggests positive effects of protein supplementation on muscle protein synthesis, muscle mass and muscle strength. However, as most studies included only small numbers of participants for short treatment periods, larger studies with longer duration are necessary to support the clinical relevance of these observations.},

METHODS One hundred ninety-five patients of a geriatric day hospital, 322 persons receiving home care (HC), and 183 nursing home (NH) residents were examined regarding BMI, cognitive (Mini-Mental State Examination), and physical function (Barthel Index for activities of daily living). Differences in Mini-Mental State Examination and activities of daily living scores between BMI groups ({\textless}22, 22-{\textless}25, 25-{\textless}30, 30-{\textless}35, $\geq$35kg/m(2)) were tested by analysis of covariance considering relevant confounders. RESULTS Activities of daily living and Mini-Mental State Examination impairments increased from the geriatric day hospital over the HC to the NH sample, whereas prevalence rates of obesity and severe obesity (35{\%},
      33{\%},
      25{\%}) decreased. In geriatric day hospital patients cognitive and physical function did not differ between BMI groups. In the HC and NH samples, cognitive abilities were highest in obese and severely obese subjects. Unadjusted mean activities of daily living scores differed between BMI groups in HC receivers (51.6$\pm$32.2, 61.8$\pm$26.1, 67.5$\pm$28.3, 72.0$\pm$23.4, 66.2$\pm$24.2, p = .002) and NH residents (35.6$\pm$28.6, 48.1$\pm$25.7, 39.9$\pm$28.7, 50.8$\pm$24.0, 57.1$\pm$28.2, p = .029). In both samples significance was lost after adjustment indicating cognitive function as dominant confounder. CONCLUSIONS In older adults the associations between BMI and physical and cognitive function were dependent on the health and care status corresponding to the setting. In the HC and the NH samples, cognitive status, as measured by the Mini-Mental State Examination, emerged as an important confounder within the association between BMI and physical function.},

DESIGN Multicenter, 1-year prospective observational study. SETTING Community. PARTICIPANTS Older adults ($\geq$ 65) receiving home care (n = 309). MEASUREMENTS Nutritional status (well nourished, at risk of malnutrition, malnourished) was classified using the MNA-SF and MNA-LF at baseline. Functional status was determined according to the Barthel Index of activities of daily living (ADLs) at baseline and after 1 year. Hazard ratios (HRs) and 95{\%} confidence intervals (CIs) of mortality were calculated for MNA-SF and MNA-LF categories using stepwise Cox regression analyses. Repeated-measurements analysis of covariance was used to examine changes in ADL scores over time for MNA-SF and MNA-LF categories. RESULTS MNA-SF classified 15{\%} of the sample as malnourished and 41{\%} as being at risk of malnutrition, whereas the MNA-LF classified 14{\%} and 58{\%},
      respectively. During the follow-up year, 15{\%} of participants died. The estimated hazard ratios (HR) for 1-year mortality were lower for MNA-SF than for MNA-LF categories (at risk of malnutrition: HR = 2.21, 95{\%} confidence interval (CI) = 1.02-4.75 vs HR = 5.05, 95{\%} CI = 1.53-16.58; malnourished: HR = 3.27, 95{\%} CI = 1.34-8.02 vs HR = 8.75, 95{\%} CI = 2.45-31.18). For MNA-SF categories, no differences in functional change were found. According to the MNA-LF, ADL decline tended to be greater in those at risk of malnutrition (7.1 $\pm$ 10.1 points) than in those who were well nourished (3.7 $\pm$ 10.1 points) and malnourished (4.9 $\pm$ 10.1 points). CONCLUSION In this sample of older adults receiving home care, the MNA-LF was superior to the MNA-SF in predicting mortality and differentiating functional decline during 1 year of follow-up.},

Besides the amount of protein, its distribution over the day is thought to affect protein anabolism. This cross-sectional study investigates the association between the amount and distribution of protein intake and frailty in older adults. METHODS In 194 community-dwelling seniors ($\geq$75~years) amount of protein intake and its distribution over the day (morning, noon, evening) were assessed using a food frequency questionnaire. Unevenness of protein distribution was calculated as coefficient of variation (CV). Frailty was defined as the presence of at least three, pre-frailty as the presence of one or two of the following criteria: weight loss, exhaustion, low physical activity, low handgrip strength and slow walking speed. RESULTS 15.4{\%} of the participants were frail, 40.5{\%} were pre-frail. Median (min.-max.) daily protein intake was 77.5 (38.5-131.5) g, 1.07 (0.58-2.27) g/kg body weight (BW) and 15.9 (11.2-21.8) {\%} of energy intake without significant differences between the frailty groups. The risk of frailty did not differ significantly between participants in the higher compared to the lowest quartile of protein intake. Frail participants consumed significantly less protein in the morning (11.9 vs. 14.9 vs. 17.4{\%},
      p = 0,007), but more at noon (61.4 vs. 60.8 vs. 55.3{\%},
      p = 0.024) than pre-frail and non-frail. The median (min.-max.) CV of protein distribution was highest in frail (0.76 (0.18-1.33)) compared to pre-frail (0.74 (0.07-1.29)) and non-frail (0.68 (0.15-1.24)) subjects (p = 0.024). CONCLUSIONS In this sample of healthy older persons, amount of protein intake was not associated with frailty, but distribution of protein intake was significantly different between frail, pre-frail and non-frail participants. More clinical studies are needed to further clarify the relation between protein intake and frailty.},

In addition the relevance of singular MNA items and subscores in this regard was tested. DESIGN Cross-sectional study. SETTING Community-dwelling older adults were recruited in the region of N{\"u}rnberg, Germany. PARTICIPANTS 206 volunteers aged 75 years or older without cognitive impairment (Mini Mental State Examination {\textgreater}24 points), 66.0{\%} female. MEASUREMENTS Frailty was defined according to Fried et al. as presence of three, pre-frailty as presence of one or two of the following criteria: weight loss, exhaustion, low physical activity, low handgrip strength and slow walking speed. Malnutrition ({\textless}17 points) and the risk of malnutrition (17-23.5 points) were determined by MNA{\circledR}. RESULTS 15.1{\%} of the participants were at risk of malnutrition, no participant was malnourished. 15.5 {\%} were frail, 39.8{\%} pre-frail and 44.7{\%} non-frail. 46.9{\%} of the frail, 12.2{\%} of the pre-frail and 2.2{\%} of the non-frail participants were at risk of malnutrition (p{\textless}0.001). Hence, 90{\%} of those at risk of malnutrition were either pre-frail or frail. For the anthropometric, dietary, subjective and functional, but not for the general MNA subscore, frail participants scored significantly lower than pre-frail (p{\textless}0.01), and non-frail participants (p{\textless}0.01). Twelve of the 18 MNA items were also significantly associated with frailty (p{\textless}0.05). CONCLUSIONS These results underline the close association between frailty syndrome and nutritional status in older persons. A profound understanding of the interdependency of these two geriatric concepts will represent the basis for successful treatment strategies.},

METHODS In 192 community-dwelling older volunteers ({\textgreater}75 years), an interview-based food frequency questionnaire was used to assess nutritional data. A Mediterranean diet (MED) score (maximum 9 points) was used to evaluate dietary quality. Frailty was defined as the presence of at least three and prefrailty as the presence of one or two of the following criteria: weight loss, exhaustion, low physical activity, low handgrip strength, and slow walking speed. Older adults without any of these attributes were defined as {\textquotedbl}nonfrail{\textquotedbl} Binomial logistic regression analysis was used to assess the risk of being frail (vs prefrail and nonfrail) in each quartile (vs lowest quartile) of the MED score. RESULTS The mean (SD) age of the participants was 83 (4) years; 41.1{\%} were prefrail and 15.1{\%} were frail. The risk of being frail was significantly reduced in the highest quartile of the MED score (OR 0.26; 95{\%} CI 0.07-0.98). CONCLUSIONS A healthy dietary pattern is associated with a lower risk of being frail. Larger, prospective and interventional studies are needed to clarify the association between dietary quality and frailty.},

    @article{Diekmann.2013, abstract = {BACKGROUND The European Society for Clinical Nutrition and Metabolism (ESPEN) has recommended the Mini Nutritional Assessment (MNA{\circledR}), the Nutritional Risk Screening 2002 (NRS), and the Malnutrition Universal Screening Tool (MUST) for nutritional screening in various settings and age groups. While in recent years all three tools have been applied to nursing home residents, there is still no consensus on the most appropriate screening tool in this specific setting. AIM The present study aims at comparing the MNA, the NRS, and the MUST with regard to applicability, categorization of nutritional status, and predictive value in the nursing home setting. METHOD MNA, NRS, and MUST were performed on 200 residents from two municipal nursing homes in Nuremberg, Germany. Follow-up data on infection, hospitalization, and mortality were collected after six and again after twelve months. RESULTS Among 200 residents (mean age 85.5 $\pm$ 7.8 years) the MNA could be completed in 188 (94.0{\%}) and the NRS and MUST in 198 (99.0{\%}) residents. The prevalence of 'malnutrition' according to the MNA was 15.4{\%}. The prevalence of 'risk of malnutrition' (NRS) and 'high risk of malnutrition' (MUST), respectively, was 8.6{\%} for both tools. The individual categorization of nutritional status showed poor agreement between NRS and MUST on the one hand and MNA on the other. For all tools a significant association between nutritional status and mortality was demonstrated during follow-up as classification in 'malnourished', respectively 'high risk of malnutrition' or 'nutritional risk', was significantly associated with increased hazard ratios. However, the MNA showed the best predictive value for survival among well-nourished residents. CONCLUSION The evaluation of nutritional status in nursing home residents by MNA, NRS, and MUST shows significant differences. This observation may be of clinical relevance as nutritional intervention is usually based on screening results. As the items of the MNA reflect particularities of the nursing home population, this tool currently appears to be the most suitable one in this setting.},

A relation between vitamin D levels and physical function (PF) is described in several studies in older adults; however, data on NHR are scarce and there is presently no information on the time course of vitamin D levels and PF in this population. The aim of the present study was to describe the 25-hydroxyvitamin D [25(OH)D] status of NHR at baseline (t1) and after 1 year (t2) to examine whether 25(OH)D blood levels are related to PF at t1 and at t2, and whether changes in 25(OH)D levels over 1 year are related to changes in PF. METHODS All NHR ($\geq$ 65 years) without tube-feeding and severe acute or end-stage disease were asked to participate. At t1 and t2 fasting blood samples were taken for the analysis of 25(OH)D serum levels and PF was estimated by activities of daily living (Barthel ADL) and measured by handgrip strength (HGS) and timed 'up and go' test (TUG). RESULTS In total, 115 residents, aged 87 (82-93) years (all data in median and 1st-3rd quartile), showed the following values for PF: ADL 50 (20-65) points, HGS 40 (30-50) kPa and TUG 26 (18-31) s. Vitamin D deficiency ({\textless} 50 nmol/l) was present in 93.9 {\%} (70.4 {\%} {\textless} 25 nmol/l) at t1 and in 71.2 {\%} (57.3 {\%} {\textless} 25 nmol/l) at t2. At t1 and at t2 a weak correlation between vitamin D level and PF (Spearman's correlation coefficient t1: ADL r = 0.367, HGS r = 0.313; t2: ADL: r = 0.247; all p {\textless} 0.01) was observed. There was no correlation between changes in vitamin D levels over 1 year and changes in PF. CONCLUSIONS Almost all NHR included in the study showed vitamin D deficiency. 25(OH)D levels were weakly correlated to PF at baseline and at follow-up, and an increase in vitamin D levels was not associated with positive effects on PF in this study.},

DESIGN Multi-centre, cross-sectional. SETTING Home care. PARTICIPANTS 296 persons $\geq$65 years in need of care (80.7$\pm$7.7 y). MEASUREMENTS Nutritional status was determined by the MNA and functional status by two questionnaires (Instrumental and Basic Activities of Daily Living; IADL, ADL) and three performance tests (handgrip strength, HGS; Short Physical Performance Battery, SPPB; Timed 'Up and Go' Test, TUG). A categorical and a covariance analytical approach were used to test for differences in functional status between MNA groups (well nourished, risk of malnutrition, malnourished). In addition, functional parameters were correlated with total MNA, a modified MNA version (modMNA), where functional items were excluded, and MNA subscales ('functionality', 'general assessment', 'anthropometry', 'dietary assessment', and 'subjective assessment'). RESULTS 57{\%} of the participants were at risk of malnutrition and 12{\%} malnourished. 35{\%} reported severe limitations in IADL, 18{\%} in ADL. 40{\%},
      39{\%} and 35{\%} had severe limitations in HGS, SPPB and TUG; 9{\%},
      28{\%} and 34{\%} were not able to perform the tests. Functional status deteriorated significantly from the well nourished to the malnourished group in all functional measures. The modMNA was weak but still significantly related to all functional parameters except TUG. The subscale 'functionality' revealed strongest correlations with functional measures. All other MNA subscales showed only weak or no associations. CONCLUSION More than one half of the seniors receiving home care were at nutritional risk and poor functional level, respectively. Malnutrition according to MNA was significantly associated to both questionnaire- and performance-based functional measures even after exclusion of functional MNA items.},

DESIGN Prospective analysis. SETTING Community, nursing home, rehabilitation. PARTICIPANTS Six hundred fifty-seven individuals aged 65 and older (75.3{\%} female; mean age 82.3 $\pm$ 7.4). MEASUREMENTS Classification agreement between full MNA score and MNA-SF scores. RESULTS Agreement between the full MNA and classification using the MNA-SFs was 84.6{\%} when the MNA-SF using body mass index (BMI) was applied and 81.4{\%} when the MNA-SF using calf circumference (CC) was applied. The highest agreement of classification was found in the community setting (90.8{\%} and 90.4{\%},
      respectively) and the lowest in the rehabilitation setting (72.4{\%} and 71.4{\%},
      respectively). Both MNA-SFs tended to underestimate nutritional status, but that was significant only for the MNA-SF with CC. CONCLUSION The modified MNA-SFs represent a valuable tool for rapid and reliable nutritional screening.},
